Health Department
It is the goal of the Health Department to assist Tribal members in achieving and
maintaining the highest level of health possible. The Health Department provides care for acute and
chronic illnesses as well as prevention education and intervention.
Eligibility for Services
You are eligible for services at the clinic if you are an enrolled member of a federally
recognized tribe. Specific eligibility requirements apply for specialty an referral care.

The Health Department consists of two major areas: Health Administration and Health
Department Clinics.
The Health Department assists with third party resource billings and receipts. Health
Administration also assists with contract health services and maintains agreements with outside providers.
Assistance is also available to help determine eligibility for and availability of alternate resources for
individual patients.
The Health Department Clinics provide the following services and/or programs:
Medical Clinic
Primary care for acute and chronic illness, provided by appointment. Must be seen by the Poarch physician
or nurse practitioner prior to receiving consideration for referred care to specialists.
Pharmacy
Fills prescriptions for eligible recipients. Prescriptions must be for medications in stock at the Poarch pharmacy.
Prescriptions must be in the clinic by 4:00p.m. to be filled on the same day.
Human Services
Provides counseling and other services for Mental Health, Drug and Alcohol Abuse, Behavior Modification, School
Liaison, and other related services.
Community Health
Provides Public Health Nursing services through clinics, in homes, and in the community. Provides home visits for
health status monitoring and reinforcement of medical instructions. Provides WIC and Nutrition services. WIC is
available to eligible recipients of the State of Alabama WIC programs. Nutrition services are available to all
eligible health care recipients.
Health Education
Available on an individual basis by appointment and through classes. Health Education is also provided as part
of the clinic visit when needed.
CDC Breast & Cervical Cancer Grant
Provides mammography and other annual women's health screening services.
Dental Clinic
Provides dental hygienist and dentist care. Service is by appointment.
Diabetic Clinic
Provides a structured program for diabetics.
Other services/programs include:
Diabetic Podiatry Clinic (by appointment and referral only), Diabetic Support Group, Diabetic Outreach,
Data entry, Medical Records maintenance, Patient Registration, and School Health programs. Other activities
sponsored by the Health Department also include Senior Olympics, Health Fairs, and Student Nursing
Preceptorships and clinical rotations.
